1. Entry




1.1 Only Australian immunisation providers may enter The Adult Immunisation Grants

Entry is free upon GlaxoSmithKline’s (GSK) receipt of the completed official entry form and entrants will be bound by these Terms and Conditions, as varied from time to time, if the application is accepted. The Grants are open to health professionals as individuals or teams working in adult immunisation in Australia.



1.2 Australian immunisation providers includes: Divisions of General Practice, Local Government/Council operated clinics, State and Territory Health Departments, Flying Doctor Service, Medicare GPs, General Practices, community nurses, public and private hospitals, Aboriginal Health/Medical Service, community health centres, hospitals, community based organisations e.g., SES and Aboriginal and Torres Strait Islander Health Workers.



1.3 GSK reserves the right to validate and verify all entries and to disqualify any person who tampers with the grants process or who submits an entry that is not in accordance with these Terms and Conditions. No responsibility will be accepted for late, lost or misdirected entries. Information included must be accurate and clearly referenced.



1.4 Criteria for entry have been decided by an independent judging panel. Individuals or teams must present, through their application proposal, a program that clearly aims to meet one of the categories described on the entry form. The judging panel reserves the right to change the entrant’s category if it fulfils the requirements of the alternate category. The decision of the Grants judging panel on criteria is final.



1.5 The purpose of The Grants is to improve awareness of, access to, and implementation of adult immunisation in Australia, and ultimately to improve adult health. It is also to share best practice and innovation in adult immunisation so that these programs can be replicated by other practices and organisations.



1.6 A maximum of four grants will be given. Each will be a one-off grant of AUD $20,000 (The $20,000 will be provided to the recipient in two instalments: $15,000 at the announcement of the successful applicant, to enable commencement of the project, with the remaining balance of $5,000 paid after a progress report provided approximately 6 months later.



1.7 Entries should be submitted according to the process outlined in the entry form. Closing date for the entries is 5.30pm (EST) Monday 19 September 2011. Late applications will not be accepted. GSK may, at its discretion extend the closing date and time. Decisions of the Grants judging panel are final. Successful entrants will be notified by mail and may be listed on the GSK website at www.gsk.com.au, together with their programs.



1.8 The Grants will be judged during September 2011.



1.9 GSK may terminate an entrant’s participation in The Grants without notice for reasons including but not exclusive to:
- failure to comply with these Terms and Conditions;
- abuses of privileges accorded to entrants; or
- provision by an entrant of misleading information or misrepresentations in connection with The Grants.



1.10 All entries may be used in promotion of The Grants, including follow-up of winning entries activities for media opportunities.



1.11 Winning entries will be announced at the Australian General Practice Network Conference, November 2011 and where appropriate, submitted for publication in medical publications. Winners will be invited to attend this conference for the official announcement of the Grants. Economy flights, registration, and accommodation (if necessary due to distance) for the day of the Grant announcement will be provided in addition to the $20,000 Grant. Only one nominated person will be eligible to receive the travel component of the Grant per winning entry.



1.12 Entrants may be contacted to be involved in media and online activities related to Grants.





2. Information Privacy




2.1 Information concerning Grant entrants, including information contained in The Grants entry form and additional supporting documentation will be held by GSK in a secure database. The database contains the following information: - name, address and telephone number(s) of entrant(s) and organisation email address(es).



2.2 All entry forms and additional supporting information become the property of GSK on submission. GSK may make copies of the entry form and additional supporting information for any purpose related to the Grants. This information will be treated in accordance with the GSK privacy policy, a complete copy of which is available at - http://www.gsk.com.au/about-us_statements_privacystatement.aspx



2.3 GSK will at all times comply with relevant Australian privacy legislation when dealing with entrants’ information. By entering, entrants consent to the information being used for that purpose. GSK will not use or disclose any entrant’s information for purposes other than administration of the Grants, without the consent of the entrants. Use of your information will be in accordance with theGSK privacy policy which is available at www.gsk.com.au/about-us_statements_privacystatement.aspx

3. Changes




3.1 GSK reserves the right to make any changes at any time without prior notice and in GSK’s absolute discretion to these Terms and Conditions and the Grants schedule. GSK will attempt to notify entrants of any such changes but will not be liable in any way for failing to do so. The GSK privacy policy will be adhered to at all times.




4. Miscellaneous Terms



4.1 GSK will not be liable for any loss, damage or expense caused by the withdrawal, suspension or termination of the Grants.



4.2 The Grants judging panel is an independent group of medical professionals from multiple medical disciplines. GSK employees may be invited to attend meetings of the judging panel in order to provide administrative support, however, they will not possess any voting rights in relation to any aspect of the Grants.
