Childhood Immunisation Awards Terms and Conditions The GlaxoSmithKline (GSK) Childhood Immunisation Awards are governed by the following Terms and Conditions. 1. Entry 1.1 Only Australian immunisation providers may enter the GSK Childhood Immunisation Awards. Entry is free upon receipt of the completed official entry form and entrants will be bound by these Terms and Conditions, as varied from time to time, if the application is accepted. The GSK Childhood Immunisation Awards are open to health professionals as individuals or teams working in childhood immunisation in Australia. 1.2 Australian immunisation providers include: General Practice Networks, Medicare Locals, Local Government/ Council operated clinics, State and Territory Health Departments, Medicare GPs, General Practices, community nurses, public and private hospitals, Aboriginal health services, community health centres and hospitals. 1.3 GSK reserves the right to validate and verify all entries and to disqualify any person who tampers with the awards process or who submits an entry that is not in accordance with these Terms and Conditions. No responsibility will be accepted for late, lost or misdirected entries. Information included must be accurate and clearly referenced. 1.4 Criteria for entry have been decided by an independent Awards Committee. Individuals or teams must present, through their application, a program that clearly meets one of the categories described on the entry form. The Awards Committee reserves the right to change the entrant’s category if it fulfils the requirements of an alternate category. The decision of the Awards Committee on criteria is final. 1.5 The purpose of the GSK Childhood Immunisation Awards is to reward innovation and share best practice in improving coverage or delivery of childhood immunisation. 1.6 A maximum of four awards will be given. Each award will consist of a one-off grant of $AUD15,000. 1.7 Entries should be submitted according to the process outlined in the entry form. Closing date for the entries is 5.30pm (EST) Friday 27 April 2012. Late applications will not be accepted. GSK may, at its discretion, extend the closing date and time. Decisions of the Awards Committee are final. Successful entrants will be notified by mail and they and their programs will be profiled on the GSK Childhood Immunisation Awards website (www.gsk.com.au/immunisationawards). 1.8 Awards will be judged during May 2012. 1.9 GSK may terminate an entrant’s participation in the GSK Childhood Immunisation Awards without notice for reasons including but not exclusive to: - failure to comply with these Terms and Conditions - abuses of privileges accorded to entrants, or - provision by an entrant of misleading information or misrepresentations in connection with the GSK Childhood Immunisation Awards. 1.10 All entries may be used in promotion of the GSK Childhood Immunisation Awards, including follow-up of winning entries’ activities for media opportunities. 1.11 Winning entries will be announced at the Public Health Association of Australia Conference, Darwin, 19-21 June 2012 and, where appropriate, submitted for publication in medical publications. Winners will be invited to attend this conference for the official announcement of the awards. Economy flights, registration and appropriate accommodation (if necessary due to distance) for the day of the Awards announcement are made available to recipients of the Awards in addition to the $15,000 grant. Only one nominated person will be eligible to receive the PHAA travel component of the Award per winning entry. 1.12 If you are granted an Award, you agree to provide a written report to GSK within 12 months of receipt of the Award outlining how the Award grant was used to improve immunisation in your area and the benefits that resulted therefrom. 1.13 Entrants may be contacted to be involved in media and online activities related to the GSK Childhood Immunisation Awards. 2. Information Privacy 2.1 Information concerning award entrants, including information contained in the GSK Childhood Immunisation Awards entry form and additional supporting documentation, will be held by GSK in a secure database. The database contains the following information: name, address, e-mail and telephone number(s) of entrant(s) and organisation. 2.2 All entry forms and additional supporting information become the property of GSK on submission. GSK may make copies of the entry form and additional supporting information for any purpose related to the GSK Childhood Immunisation Awards. This information will be treated in accordance with the GSK privacy policy, a complete copy of which is available in the GSK Privacy Policy. 2.3 GSK will at all times comply with relevant Australian privacy legislation when dealing with entrants’ information. The information held in the database will only be made available to third parties involved in administering the GSK Childhood Immunisation Awards. By entering, entrants consent to the information being used for that purpose. GSK will not use or disclose any entrant’s information for purposes other than administration of the GSK Childhood Immunisation Awards, without the consent of the entrants. Use of your information will be in accordance with the GSK privacy policy which is available at www.gsk.com.au/about-us_statements_privacystatement.aspx You may access the information GSK holds about you by writing to: The Privacy Officer, GlaxoSmithKline Australia Pty Ltd, PO Box 168 Boronia VIC 3155. 3. Changes 3.1 GSK reserves the right to make any changes at any time without prior notice and in GSK’s absolute discretion to these Terms and Conditions and the Awards schedule. GSK will attempt to notify entrants of any such changes but will not be liable in any way for failing to do so. The GSK privacy policy will be adhered to at all times. 4. Miscellaneous Terms 4.1 GSK will not be liable for any loss, damage or expense caused by the withdrawal, suspension or termination of the GSK Childhood Immunisation Awards. 4.2 The Awards Committee is an independent group of medical and health professionals from multiple disciplines. Relevant GSK employees may be invited to provide input or support to the Awards Committee, however, they will not possess any voting rights in relation to any aspect of the GSK Childhood Immunisation Awards. |
